Closing or Taking Leave from a Medical Practice

The College expects physicians to take reasonable measures to reduce the impact of their closure of or leave from practice on the continuity of care for patients.

When contemplating closing or taking leave from your medical practice, you must comply with the College’s Standard of Practice.  As per the Standard of Practice, you are expected to notify your patients in advance of your planned closure of or leave from practice. In most circumstances, the College recommends that you should provide your patients with a minimum of 90 days’ notice.

Please review the College’s Standard of Practice on Closing or Taking Leave from a Medical Practice, as well as the Frequently Asked Questions, Physician Checklist, and the College’s Standard of Practice on Medical Records Documentation & Management, so that you are familiar with the College’s expectations.

You are required to notify the College in advance of your planned closure of or leave from practice by completing the appropriate online form below.
